Dear Healthcare Provider,

The Illinois Department of Public Health (IDPH) Division of Patient Safety and Quality is inviting outpatient facilities to participate in the Precious Drugs & Scary Bugs Campaign. This statewide initiative aims to increase appropriate use of antibiotics, particularly for upper respiratory infections evaluated in the outpatient setting.

Campaign participation entails:
1. Campaign sign-up
   ---Facility interest form: completed by the leader of each facility/practice (e.g., Medical Director)
   ---Provider commitment form: Each individual provider (i.e., prescriber) at participating facilities should complete the commitment form below.
2. Baseline and follow-up survey: completed by individual providers at participating facilities
3. Display poster-sized commitment letter in exam rooms that indicates provider commitment to prescribing antibiotics judiciously. Poster will feature provider photograph and signature.
4. Optional activities
---Participate in educational offerings related to antibiotic use.
---Facilities are also encouraged to track data on provider antibiotic prescribing.
