Care Provider Questionnaire
The goal of participating in your child's speech-language therapy was to provide you with tips and activities you can do at home to facilitate language development, model techniques that will aid in your child's language development, coach you through using those techniques, instill an understanding of the importance of your role in your child's language development, and introduce materials from the Resource Center that address language development. We would like you to tell us how well we did what we set out to do. Please answer the questions based on your experience.
