Maine Veterinary Medical Association Client Satisfaction Survey

Your veterinarian values your patronage. In order to serve you better, he/she is asking you to participate in the following short client satisfaction survey. This survey is being conducted by the Maine Veterinary Medical Association. All responses will be kept strictly confidential. Your veterinarian will receive general results but will not have access to your completed survey or your answers.

Upon completion of the survey, you will be eligible to enter a drawing for $250. Participation in the drawing is optional. Any contact details provided will only be used for the prize draw and will not be disclosed to anyone (including the veterinarian).
